چکیده

During the civil war in ex-Yugoslavia, a large number of patients was treated at our clinic for injuries to the lower third of the face.Approximately, one third of them required reconstruction of large and small bone and soft tissue defects of the mandibula. The incidence of war injuries of the maxillofacial structures from 8% to 17%, according to the literature (1). In the ex-Yugoslavia civil war the percentage was 10% (2). Since 1993, we have been using free flap reconstruction for large defects of the lower third of the face: scapular, radial, forearm and fibular. Microsurgical free flaps have been extensively used in the reconstruction in this region (3, 4). A growing body of evidence suggests that microvascular osteocutaneous free tissue transfer is the most sophisticated method to reconstruct composite defects of the lower third of the face (5). In peace time, it is also one of the most reliable methods ranges to perform surgery in a single act for tumor ablation in the distal portion of the face, with immediate mandibular reconstruction. The purpose of this study was to demonstrate the results with microvascular free osteocutaneous flaps in the reconstruction of mass mandibular defects and facial soft tissue defects caused by war injuries.

Reverse-flow scapular osteocutaneous flap for head and neck reconstruction.

BACKGROUND The scapular osteocutaneous flap can be used for the reconstruction of a variety of head and neck defects. In this study, we describe use of the reverse-flow scapular osteocutaneous flap in the setting of a vessel-depleted neck. METHODS AND RESULTS A retrospective review of patients who underwent scapular osteocutaneous free flap reconstruction over a 5-year period revealed 3 patie...

Oromandibular Reconstruction: The History, Operative Options and Strategies, and Our Experience

Oromandibular reconstruction resulting from resection of benign tumor, malignant cancer, osteomyelitic or osteoradionecrotic mandible remains a challenge for plastic surgeons today. At present, fibula osteocutaneous flap is the perhaps most commonly used technique for oromandibular reconstruction because of its potential for contouring, immediate dental implant placement, and favorable donor si...

Perforator Flaps for Reconstruction of Lower Limb Defects

BACKGROUND Reconstruction of soft tissue defects in the lower third of the leg remains challenging. Anatomical constraints limit the local options available for complex defects especially lower third of leg. Local flaps based on perforator vessels are raising interest in reconstructive surgery of the limbs. We present our experience with perforator flaps for reconstruction of soft tissue def...

Functional outcomes of fibula and osteocutaneous forearm free flap reconstruction for segmental mandibular defects.

OBJECTIVE/HYPOTHESIS To demonstrate that the osteocutaneous radial forearm free flap provides equivalent functional outcomes and improved morbidity compared to the fibular free flap in mandibular reconstruction. STUDY DESIGN Retrospective review. METHODS There were 168 patients requiring free flap reconstruction of segmental mandibular defects between January 2001 and December 2008. Mean fo...

Comparison of radial forearm with fibula and scapula osteocutaneous free flaps for oromandibular reconstruction.

OBJECTIVE To compare our experience with the osteocutaneous radial forearm free flap (group 1) (n = 108) with other commonly used osteocutaneous free flaps (group 2) (n = 56) such as the fibula and scapula in single-stage oromandibular reconstruction. DESIGN Retrospective case review. SETTING Tertiary-care academic medical center. PATIENTS One hundred sixty-three consecutive patients who ...
